Commissioning extra care

We are interested in hearing from commissioners who have purchased domiciliary care as a block contract for an extra care scheme (or schemes), including the provision of overnight care (waking and sleeping). Are there examples of contracts which can be shared? How were unit costs and payment methods agreed, and what monitoring structures have been used/are successful to ensure care hour delivery is kept within agreed limits?
