Day care and other services within an extra care scheme

We are just developing are first Extra Care Scheme but are grappling with the issue of how we replace all of the services currently provided within a Local Authority Residential Care Home within an Extra Care Setting, in particular our traditional day-care, but also respite, meals on wheels etc. Our instincts tell us that as more flexible and appropriate services become available then the demand for traditional services will diminish so we are reluctant to simply build a new day centre within the new scheme, which may become obsolete in the near future.

How have others tackled this reprovision issue if they have replaced traditional residential homes and day care with Extra Care?
