Eligibility Criteria and Assessment Procedures for Extra Care Housing

Does anyone have copies of eligibility criteria and assessment procedures for admission to Extra Care Housing which they would be willing to send me copies of?

I am particularly interested in those which have been developed with housing authorities and which are operated jointly. Any comments on how well (or not) they work would be helpful.

Hi Liz, I hope the attached eligibility criteria in use in Southampton is helpful to you. The criteria and route into the Extra Care Project we have in Southampton has been jointly agreed between Housing Services, the landlord, Social Services and the independent Care provider etc.

I manage a serivce in Liverpool which is a partnership between the local authority (housing and social care) RSL's and the PCT. We already manage a commona allocation policy for standard sheltered and for adapted property. From 14/2/05 we have adopted a common assessment and allocations policy for extraw care schemes too. I attach a copy of our policy document for your information.

There is a shared allocation policy on the Suffolk CC website www.suffolkcc.gov.uk , (go to social care go to housing I think) that we use for all supported housing services in Suffolk incl. VSH and Extra Care. People say that it works very well, keeps voids low and has everybody signed up to the allocation process.

Eligibility criteria is a standard 4 hours personal care across all the services. Happy to talk to whoever if they want to ring me....

we as the housing provider worked up eligibilty and allocations criteria and procedure with our social services partners. Will e-mail you a copy this week. One of the issues that is apparent is that it is not streamlined enough between housing and social services and also Social Services assessment staff not understanding extra care housing enough to make a good assessment as to whether extra care is most apporpriate.
