I am currently working with colleagues to write an admissions policy for a new extra care scheme and would be interested to see what people in other localities are currently doing in this area?

Any copies of policy's/criteria/hints and tips would be appreciated.

I have attached Sunderland City Council's nominations /allocations policy for our extra care schemes which I hope is helpful. I have a copy of the self assessment application which is a 'dual' application form in terms of 'housing register' requirements and care and support information which prevents the applicant having to complete more than one application form. Please let me know if you would like a copy and I can send this through to you.
