Extra Care Policy

I'm just in the process of setting up a new Extra Care scheme and working on producing an Extra Care Policy. There is a good template on Housinglin which we're using as a guide, just wondered if there was anyone who had an extra care policy their happy to share as an example of policy currently being used.
