Extra Care Resident surveys

We would like to carry out a comprehensive survey of tenants in our extra care schemes in Salford. We would like to capture views of housing, care & support. (to avoid having to carry out multiple surveys) Has anyone any examples of such surveys and would you be prepared to share them?
