Extra Care - models of care and support

Hi, we are reviewing models of care and support to rollout for a scheme currently under development and future schemes. I am looking for colleagues willing to share their local models.

All extra care schemes need to find ways of delivering four distinct services:
•Housing Management
•Personal Care
•Housing Related Support
•Meals

The meals service is generally a standalone service funded by residents when they purchase their meals.

The other three services can in theory be delivered by three different organisations (i.e. Landlord / Registered Domiciliary Care Provider / Support Provider) but in different schemes different organisations deliver a combination of these services.

A complication arises from one of the features of extra care housing - the need to provide a 24 hour staff presence. This "building cover" does not fall neatly into any of the above categories of Housing Management, Personal Care and Housing related Support.

Also our current local models separate out individual needs/provision but result in complicated arrangements for paying providers and charging tenants. In the light of the personal /individual budgets agenda we want to work out the best model to promote across the county

Would any forum members be prepared to share existing or planned model (s) and concerns or issues?

We in Blackburn with Darwen also have a scheme in development and would also be interested in hearing from colleagues on this issue.

Hi Marie
Hope this model helps. In Sunderland we are working in partnership with Housing 21 to provide three extra care schemes.

Housing 21 provide the landlord/housing management function between the hours of 9 - 5 - Monday to Friday. The Council's Care Team provide 24/7 care and support services 365 days with the support of telecare provision.

We have a social enterprise / Community Interest Company who run the restaurant which is open to residents and members of the public visiting the scheme. Eating at this restaurant is not a condition of tenancy and provides the residents with a choice of eating in their own apartments; at the restaurant or elsewhere.

The 24 hour staff presence is covered by the care team as there is waking night cover arrangements in place and due to the operation of the building they are available to provide this cover in the absence of the court manager. The care staff have handsets linked to the assistive technology and can adequately manage the building on this basis.
