Qualifying care criteria for extra care sale units

We are developing a 54 unit extra care scheme with some of the units for sale. We need to provide the estate agent with clear and transparent qualifying care criteria so they can determine who will be eligible for buying these properties. Would be very interested in how others with similar schemes have achieved this and if you have a pro-forma that you could share? I would really appreciate your assistance with regard to this.
