Supporting People delivery changes for older people

Here in Devon we have completed the sectorial reviews for SP. As a result we have contracts being renewed for 12 months during which time we need to be advising our providers of the preferred delivery method and helping them change their services. We have a large County with numerous providers of SP and 8 District Councils. Has anyone developed/ing an Older Persons and/or Physical Disability Housing strategy which includes a method of pushing forward the SP review outcomes? Anyone got a blueprint? All comments welcome.
