extra care housing-strategic approach

Hello,

We are currently reviewing our Older People’s Housing Strategy in Gwynedd Council, and will be looking to reduce our dependency on residential care and probably suggest increasing our Extra Housing Care provision. We are collating data on population trends and predictions and the current provisions and demand etc.

I was wandering if anybody is happy to share good practice around what they have used for reasons for, why and how they have reduced their usage of residential care and what they found feasible to reduce and the process that they undertook?

Hi Lowri

I used home care data- looking at how many were in each post code area and which post code contained the biggest packages (by hours) which gave me an indication of where the greatest existing need was. Some of this could be better delivered in extra care schemes.

Also, i don't know what method you have in your authority but we have a panel who assess new assessments for local authority placements (usualy coming out of hospital)- this gave me an indication of the type of need, especially if new extra care will be a replacement for residential care.

The POPPI website is alway good for data and has just been updated too!
