Anticipatory Care: Interventions Framework

NHS England

This Intervention Framework (IF) sets out key areas in which interventions may be effective to address the issues identified during assessment and care planning.

It gives an indication of the types of services and supports that might need to be in place to address the AC requirements of individuals.

Jeremy Porteus, CEO at Housing LIN, commented:

"The Housing was delighted to be involved with developing and providing supporting information for this Framework. It is a particularly useful resource for those involved in planning, commissioning, social prescribing or providing at home care and support. It recognises the importance of access to housing and the suitability of the home environment in tackling health inequalities and signposts to relevant tools and interventions where accessible design, adaptations, the use of technology and related services can qualitatively improve people’s care pathways."
