Refreshing our Home From Hospital Map

As part of our commitment to the national Health and Housing Memorandum of Understanding, we have added several new (pre-coronavirus) projects on our Home from Hospital interactive map for England.

This map, developed by Foundations and the Housing LIN, highlights examples of where housing organisations and local services are, or have been actively supporting hospital discharges and helping to get people home. Scrolling over a map of England, you can find each project geographically or by using the filter to search the following four categories:

  • Co-located / Embedded Caseworkers + Community Based practical
  • Co-located / Embedded Housing Support within Discharge team
  • Community based practical Housing support
  • Step Down Housing Provision

The interactive map is also referenced as a useful tool in the government’s High Impact Changes Model which now also specifically covers housing (see Change 9).
