Shared outcomes toolkit for integrated care systems

Following the creation of Integrated Care Systems (ICSs), the Department of Health and Social Care has published a toolkit to support place partnerships to develop their own shared outcomes as a powerful means of bringing organisations together from across the health and social care system and making integrated services a reality.

The toolkit includes case studies of good health and social care practice, suggestions for overcoming challenges, and example frameworks.

The toolkit complements the work many ICSs are already doing to develop their shared outcomes, and has a fourfold purpose of:

• Improving outcomes in population health and healthcare
• Tackling inequalities in outcomes, experience and access
• Enhancing productivity and value for money
• and helping the NHS support broader social and economic development,

However, there are only a few specific mentions of housing. Nonetheless, the toolkit provides housing providers an understanding of how place partnerships are being developed across health and social care systems and how best housing can engage and contribute to the outcomes being sought. For example, consideration could be given to the health and housing Memorandum of Understanding that some localities have developed to achieve this against local priorities.
