I work for a LA and need to put together a contract or SLA, and a license agreement for some designated properties, to be used as short term reablement properties. We will mange these properties on behalf of a health commissioning team. It will be for people who are ready to leave hospital, but are unable to return home (due to possible adaptations needed to their own property). these properties will be within a sheltered housing scheme. I am keen to hear from anyone who has a similar arrangement on how this works in practise and what contracts/license agreements they have in place. Thank you.
