Evaluating integrated and community-based care: how do we know what works?
This report from the Nuffield Trust presents the key learning from over 30 community-based interventions designed to reduce emergency hospital admissions. It should provide useful learning for the new health and social care integration 'pioneer' sites (opens new window) that are expected to be appointed by the Department of Health by September 2013 (see 'Integrated Care: Our Shared Commitment' (opens new window)). Of note are:

- Whole system demonstrator trial (opens new window) of the use of telehealth and telecare in four pilots, and
- Department of Health-funded integrated care pilots
