High Impact Change Model for managing transfers of care

The Better Care Fund Support Programme commissioned an updated High Impact Change (HIC) Model to assist local system partners in enhancing health and wellbeing, reducing avoidable hospital stays and promoting innovative interventions.

The model provides a practical approach to support local health and care systems in managing individual journeys and discharges, incorporating a specific change (No10) focused on housing and related services.
