Integrating Extra Care: partnership working in Staffordshire

This Housing LIN Case Study No 47 looks at an example of successful joint working by eight partner organisations at Mill Rise, a new-build Extra Care scheme in Staffordshire (see document below). Managed by Aspire housing, the 60 unit scheme is integrated with Millhouse Primary Care Centre.

Six years since it opened, Community Health Partnerships have produced a short paper outlining the health and social benefits of co-located primary care services and extra care housing (see link below).
