Intermediate Care Fund in Wales 2016-17

The Welsh Government has announced £60m funding to continue projects which have proven to be successful in helping older and vulnerable people remain in their own homes. This £40m increase from the 2015 fund aims to maintain the avoidance of unnecessary hospital admissions and delayed discharges.

However, it is now extending the scope of the integration agenda to look at the development of integrated care and support services for other groups of people.
