On the Pulse: How housing is critical to better health and social care for older people
This report from the National Housing Federation, supported by the Housing LIN, highlights the critical role which housing providers play in improving health and social care outcomes for older people. It follows the NHF's earlier report, Breaking the Mould, about re-visioning and broadening the housing options available to people in later life.
On the Pulse explores how housing associations can work with health and social care commissioners to enable older people to manage changes in their health, to maximise independence and reduce the need for more costly care and support. It features analysis and six innovative case studies spanning support for patients with dementia, telecare, home from hospital services, short-term intermediate housing and end of life care.
The report is designed to help housing providers to engage with commissioners and other local partners about a range of housing solutions which the sector can offer older tenants to enable them to live well at home.
