The Older People's Commissioner for Wales, an independent voice and champion for older people throughout Wales, has developed a guide on hospital discharge for older people and their families. It provides a range of helpful information about what older people should expect when they are ready to leave hospital.

Answering eleven common questions that those leaving hospital and their families might have, the guide also includes information about people’s rights. There is also a list of contact details for organisations in each local area that can provide help and support, as well as a glossary of related terminology.
