Housing LIN to develop new demand modelling tool for housing for an ageing population

The Housing LIN is pleased to announce that through a partnership with UK Research and Innovation (UKRI), we are developing a new, comprehensive demand modelling tool for housing suited to an ageing population.

One aim of the new modelling tool is to provide local authorities and their partners in the UK with robust and reliable evidence of demand which they can use in relation to housing and social care commissioning and delivery plans as well as their Local Plans.

High Impact Change Model for managing transfers of care

The Better Care Fund Support Programme commissioned an updated High Impact Change (HIC) Model to assist local system partners in enhancing health and wellbeing, reducing avoidable hospital stays and promoting innovative interventions.

The model provides a practical approach to support local health and care systems in managing individual journeys and discharges, incorporating a specific change (No10) focused on housing and related services.

Access Denied: Older people’s experiences of digital exclusion in Wales

A new report from the Older People's Commissioner for Wales warns a growing number of older people in Wales are at risk of social exclusion and being left behind as the use of digital technology continues to play an even greater role in our everyday lives.

The Commissioner offers a series of recommendations to both the Welsh and UK governments, as well as to other public bodies and the private sector, including utilities and telecoms providers.

Proactive care to provide care and support for people with moderate or severe frailty

NHS England has issued guidance on developing a more consistent and integrated approach to proactive care across the country for people living at home with frailty.

It aims to utilise existing resources, evidence and best practice to improve support for those with moderate or severe frailty.
